Medical consent

I consent for first aid treatment to be administered to the named player in my absence if I cannot be contacted on any of the above numbers. In accordance with the club’s child protection policy, I consent to my child being conveyed directly to hospital in the event that he/she requires treatment for injury.

The email address you have provided will be used to inform you about A2Pro news and events. It may also be used to send you information.

Photographs

Pictures of the teams, squads, players and or managers / coaches with players maybe put on any future website

Participation

The parent / guardian consent to the named player being registered with A2Pro to train or play in sanctioned FA associated football matches.

We (the named player and I) will abide by the constitution, rules and codes of conduct as laid down by A2Pro. I understand that failure to do so could result in the registration being withdrawn.

I agree to take all reasonable care of any kit belonging to the club.

I acknowledge that I will be responsible for payment of any disciplinary fines incurred as a result of the named players, either from the Berks and Bucks football association or any other governing body or association.

I acknowledge that team selection rests with the team manager, that participation in any A2Pro activity is at my own risk, that the club does not provide insurance cover for claims arising from player on player incidents. I retain responsibility for the named player at all times or arrange another responsible adult to do so.

GDPR

All data collected from parents via contact forms or other means is used for the purposes of keeping parents informed of A2Pro training and events.
